By offering consistent, real-time data, ConnectedCare™ enables a comprehensive, long-term view of your health for your physician. This approach not only helps doctors make more informed decisions but also empowers patients to take a more proactive role in their own care.
With ConnectedCare™, you’re giving your healthcare provider the tools they need to monitor your health trends closely and adjust treatment plans accordingly, ultimately enhancing your overall quality of life.
- We manage ordering and delivery of your device and provide education to properly configure your devices.
- We triage any readings outside safe parameters and alert your physician as needed.
- Our clinical coaches meet with you at least once per month to guide you on your wellness journey.
- You'll get reminders via a push notification to take your medications at the appropriate time.
- We will track your vital signs a minimum of 16 times a month per 30 day period using the MyWellness Connected app.
