ConnectedCare™ is designed to support Medicare patients who are managing at least one chronic condition, such as diabetes, hypertension, or heart disease. Our innovative, FDA-approved devices track critical health metrics—like blood pressure, glucose levels, and heart rate—on a daily basis. By offering consistent, real-time data, ConnectedCare™ enables a comprehensive, long-term view of your health for your physician. This approach not only helps doctors make more informed decisions but also empowers patients to take a more proactive role in their own care.

With ConnectedCare™, you’re giving your healthcare provider the tools they need to monitor your health trends closely and adjust treatment plans accordingly, ultimately enhancing your overall quality of life.

We manage ordering and delivery of your device and provide education to properly configure your devices.
We triage any readings outside safe parameters and alert your physician as needed.
Our clinical coaches meet with you at least once per month to guide you on your wellness journey.
You'll get reminders via a push notification to take your medications at the appropriate time.
We will track your vital signs a minimum of 16 times a month per 30 day period using the MyWellness Connected app.
