Is your Medicare plan changing or ending?
A change to a Medicare Advantage or drug plan does not mean Medicare itself is ending. Start with the notice you received, the name of your plan, and the year the notice covers.
MEDICARE QUESTIONS
Ask Grace a question
Grace is Community Wellness’s automated assistant. Get an explanation of Medicare terms or help preparing questions for Medicare, your plan, or a SHIP counselor. Grace does not choose a plan for you or enroll you in coverage.

Check what the notice actually says
- Find the plan’s full name and, if shown, its contract and plan numbers. Plans with similar names may offer different coverage.
- Look for whether the plan is changing benefits, leaving your area, or ending its Medicare contract. An Annual Notice of Change describes next year’s changes; it is not always a termination notice.
- Note the date your current coverage ends and any instructions about choosing new coverage.
- Call the member-services number on your plan card or notice. Ask: ‘Will my exact plan still be offered where I live next year?’
Ask which enrollment period applies
Medicare’s annual Open Enrollment runs October 15 through December 7, with changes generally starting January 1. Other enrollment periods can apply when a plan ends or circumstances change. Your notice and Medicare can help identify the dates that apply to you.
If you are considering Original Medicare and a Medigap policy, ask about your federal and state rights before making a change. The ability to buy a policy without medical underwriting depends on the situation. Do not assume the annual Medicare enrollment period gives everyone the same Medigap rights.
Before you choose new coverage
Grace can explain terms and help you prepare questions. These pages do not look up your individual plan’s termination status or calculate a personal deadline.
- Check your prescriptions, pharmacy, doctors, and hospital against the specific coverage you are considering.
- Ask how and when the new coverage would begin, including prescription drug coverage.
- Keep copies of notices and confirmation numbers. Ask Medicare or a SHIP counselor to explain anything you are unsure about.
