COMMUNITY WELLNESS · MEDICARE RESOURCES
Programs that may help with everyday needs
Help may be available beyond your health plan. Different programs address medicines, food, transportation, energy bills, and support at home. Start with the need that matters most to you, then contact the agency that runs the program.
MEDICARE QUESTIONS
Ask Grace a question
Grace is Community Wellness’s automated assistant. Get an explanation of Medicare terms or help preparing questions for Medicare, your plan, or a SHIP counselor. Grace does not choose a plan for you or enroll you in coverage.

Places to start
- Medicines and Medicare costs: ask about Extra Help, Medicare Savings Programs, and other cost assistance through Medicare, Social Security, and your state.
- Food and meals: ask your local aging-services agency about meal programs, and check your state’s food-assistance information.
- Transportation and support at home: ask local aging services and your health plan what is available and what the rules are.
- Energy bills and other living expenses: use government benefit directories to find programs and state contacts.
- Veterans and family benefits: check VA resources for the benefits that may apply to your situation.
- Caregiver support: ask about respite, training, community programs, and any programs that may pay a caregiver.
Ask these questions before applying
- Who runs the program, and is this its official contact information?
- What are the eligibility rules and what information would I need to provide?
- Is help available now, or is there a waitlist or application window?
- Does the benefit need to be renewed, and how will the agency notify me?
- Could this interact with other benefits I already receive?
Find the right agency
The Eldercare Locator connects older adults and families with community resources. USA.gov provides links to government benefits and state agencies. Program administrators make eligibility decisions; a general guide or automated conversation cannot guarantee a benefit.
These pages do not collect financial information, rank benefits by savings, file applications, or monitor renewals. Grace can help explain unfamiliar terms and suggest questions to ask.
